O documentário é uma crítica à forma como os americanos se alimentam. Acompanha Morgan Spurlock em seu projeto de passar 30 dias comendo apenas itens do cardápio do McDonalds. O filme apresenta também entrevistas com consumidores, médico e representantes da indústria alimentícia sobre as redes de fast food e os hábitos alimentares dos norte-americanos. O título do filme (Super size) refere-se à prática estimulada pelo Mcdonalds de oferecer ao consumidor uma refeição de tamanho supergrande em troca de alguns centavos a mais no preço.
